Your Health Summary allows authorised medical services to easily access your up to date health information so you can continue to get the best care. If you can’t visit your normal medical centre, or need emergency care - Your Health Summary is available 24/7.
To see what information is held about you in Your Health Summary record you can check the patient portal with your medical centre. You can also request your health information from your medical centre directly. There may be differences if some information is not shared into the Your Health Summary record.
